Blood Tests for Colitis and Crohn's Disease: an Introduction

Blood tests for colitis and Crohn's disease are a relatively new and exciting development that have added significantly to the screening, diagnosis and management of ulcerative colitis and Crohn's disease. Differentiating the two may allow better predictions regarding responses to medical treatments, decisions regarding surgery options and the risks of various complications. Antibodies to various proteins including Baker's or Brewer's yeast (saccharomyces cerevisiae) and bacteria like Escherichia. coli (E. coli) are present in the blood of many people with Crohn's disease but rarely in normal people. Antibodies to a normal cell component, a nuclear protein, is present in most people with ulcerative colitis, a few people with Crohn's whose colitis behaves more like ulcerative colitis than Crohn's, and rarely in normal people.

Antibody tests or serologic markers are blood tests looking for markers of diseases. The serologic markers or antibody tests for ulcerative colitis and Crohn's disease are pANCA and ASCA, OmpC, and CBir1 Flagelin respectively. The latter three blood tests for Crohn's are only available through one laboratory, Prometheus Laboratories, Inc.

Ulcerative colitis is a chronic inflammatory bowel disease (IBD) of unknown cause that only involves the colon. It affects the superficial lining of the colon and rarely causes bowel obstruction (blockage) or perforation (rupture) but frequently causes severe bloody diarrhea, blood in the stool, weight loss, abdominal pain, as well as joint aches or arthritis, skin rashes, eye irritation and occasionally a severe liver disorder known as primary sclerosing cholangitis that can lead to cirrhosis and liver cancer. Ulcerative colitis can be cured by complete removal of the colon but not Crohn's disease.

Crohn's disease can also cause colitis but usually also affects the very end of the small intestine called the ileum (ileitis or regional enteritis). When Crohn's affects only the colon it may be difficult to distinguish it from ulcerative colitis though Crohn's tends to affect the colon in a patchy manner whereas ulcerative colitis is continuous. Crohn's can affect the gastrointestinal tract anywhere from the mouth to the anus and is not curable by removing the colon. It is also frequently associated with bowel strictures (constrictions) causing obstruction that may require surgery. It also may be associated with fistula that are abnormal connections of the intestine to other organs and the skin or it can result in abscesses or perforation requiring surgery It is important to distinguish Crohn's disease from ulcerative colitis since medical treatments and surgical approaches may differ and the types of complications that can occur can be much different.

Traditionally, the diagnosis of ulcerative colitis and Crohn's disease is highly accurate by the appearance of the colon on colonoscopy or x-rays that confirm the presence or absence of involvement of other parts of the intestinal tract. Diagnosis is confirmed by a typical pattern of inflammation of the intestine lining as seen under the microscope on tissue obtained by biopsy during colonoscopy. However, before blood tests were available about 10% of people with IBD were diagnosed as having an indeterminate colitis because the biopsies could not distinguish between the ulcerative colitis and Crohn's disease.

The blood tests currently available are pANCA, anti-ASCA, anti-OmpC, and anti-CBir1 flagelin antibodies. pANCA is the peripheral anti-nuclear antibody. It is an abnormal antibody to nuclear protein of cells and is highly sensitive and specific for ulcerative colitis. The pANCA anbibody has been further divided into subsets by Prometheus Laboratories Inc. Neutrophil-specific pANCA ELISA (NSNA) is positive in the majority of people with ulcerative colitis (UC) and a small subset of people with Crohn's disease that have disease characteristics more like UC. Immunofluorescent cellular staining of neutrophils (NSNA IFA) and enzyme Dnase testing (NSNA DNase sensitivity) is also done as part of the Prometheus IBD Serology 7. The latter test when present in high levels is significantly associated with development of inflammation of the rectal pouch (pouchitis) created when someone has their entire colon removed for ulcerative colitis that does not respond to medical treatment.

ASCA is anti-saccharomyces cerevisiae antibody. Saccharomyces cerevisiae is Brewer's or Baker's yeast. Crohn's patients have a high prevalence of abnormal antibodies to this yeast. Some have suggested that another yeast, Candida albicans, somehow plays a role in this abnormal response. A few people with celiac disease have this antibody present in their blood in the absence of signs of Crohn's disease. OmpC is the abbreviation for an antibody that develops in many Crohn's patients to the outer membrane porin protein of the bacteria E. coli though that bacteria is not thought to be the cause of Crohn's disease. Just recently Prometheus Laboratories added antibody testing for a specific protein on bacteria that constitutes the flagelin or hair like structure on certain bacteria enabling movement and attachment of bacteria in the intestine called CBir1 flagelin.

Future blood tests may include antibodies against certain sugar (mannose) residues in the cell wall of the yeast saccharomyces cerevisiae. Anti-laminaribioside and anti-chitobioside antibodies were recently reported to be present in Crohn's patients who were anti-ASCA negative possibly further strengthening the ability to distinguish them from people with ulcerative colitis. This is also interesting because of suspicions and the lay public interest in the role of sugars or glycans and yeast in IBD. In particular the reports in lay literature of success of carbohydrate specific diet in IBD.

If you have a diagnosis of ulcerative colitis or Crohn's disease these blood tests may be very helpful in your treatment. If you have unexplained abdominal pain, diarrhea, or blood in your stools then these tests should be considered. If you have a diagnosis of irritable bowel syndrome, these tests may exclude ulcerative colitis and Crohn's disease. Since as many as 10% of people with ulcerative colitis and Crohn's disease may also have celiac disease, celiac blood tests should also be considered. Lactose intolerance is also common in IBD, IBS and celiac disease.

High Blood Sugar - What Are Considered Normal Blood Glucose Levels?

The normal blood sugar level for a person depends upon when the test for blood glucose is performed as well as whether an individual is diabetic or not. When performed in the morning before the first meal - fasting blood sugar - the normal range is from 70 to 100 milligrams per deciliter. This test is performed without food for the six to eight hours previous and should not be confused with either the preprandial test. A preprandial test, one performed two hours before a meal, will have a range from 80 to 110 milligrams per deciliter. A postprandial test is performed after a meal, typically two hours after eating, and should not exceed 145 milligrams per deciliter.

However, these levels are not those recommended by the American Diabetes association for diabetics. Preprandial (including fasting), the blood sugar is expected to be from 70 to 130 milligrams per deciliter. The postprandial glucose level should be below 180 milligrams per deciliter. Diabetics, either because of insulin resistance or reduced insulin production, are unable to metabolize glucose as well as non0diabetics, hence the higher normal levels.

The blood glucose level, even for diabetics, is actually a rather narrow range, however levels out of range can be harmful and even life-threatening. A blood sugar level below 70 milligrams per deciliter is termed hypoglycemia, and below 60 to 65 milligrams per deciliter, an individual will begin to feel and show symptoms - shakiness, hunger, and racing of the heart among others. As blood sugar drops below 50 and further, mental function is affected and progressively deteriorates leading to unconsciousness and seizures.

At the other extreme is hyperglycemia, a glucose blood level above 180 milligrams per deciliter. However, symptoms - especially frequent or excessive hunger, thirst, or urination - often aren't experienced until the level reaches 270 to 360 milligrams per deciliter. It should be noted that a chronic, sustained blood glucose level of just 125 milligrams per deciliter can lead to organ damage.

Before actually conducting a self-test, the diabetic must follow these SOPs-

1. Wash your hands carefully and completely with soap and water.

1. Do not prick the tip of the finger. This is really painful. Always prick the side of the finger. Another thing to remember, only prick fingers. Never prick your toes. Not only is this inconvenient, this will also increase the possibility of infection.

2. Do not ever squeeze your fingers if the blood refused to come out. Let the blood come out naturally. If it does not, let gravity do its work. Never force the issue. Ask your doctor what you can do to encourage sufficient blood flow.

3. Do not prick the same finger every time. This will stress the area out making that specific finger sore.

4. Do not use the same finger lancet over and over again. This is unhygienic and incredibly stupid. Bacteria have already accumulated in the used lancet. Reusing the same lancet encourages infection.

Do not fool yourself by not eating and exercising more when you know you will check your blood glucose level that day. This will lead to inaccurate results and is not really helpful in helping you control diabetes.

We all experience blood clots from time to time, when we are bleeding; our body creates blood clots to stop the bleeding. However, when a blood clot prevents the natural flow of blood, blocks a vein, or blocks an artery, it becomes harmful and extremely hazardous to our health. Am I at Risk?

Some people are at an increased risk of developing a blood clot. For example, those who suffer from any type of cancer have a high risk of developing blood clots. It is for this reason, that cancer patients generally cannot travel far distances in cars, trains, or airplanes, without having to stop now and then to walk and get their flow of blood moving again.

Some other people who are at increased risk includes those who are under a physician's order for bed rest or have decreased mobility. Those who suffer from obesity, have a history of DVT, or have suffered fractures have an increased risk as well. Elderly people, smokers, women taking birth control, those with arteriosclerosis, and those undergoing chemotherapy also have higher risks of developing a blood clot.

It is important to watch for the signs of a blood clot, as well as understand if you are at a high risk of developing one.

Causes of High Diastolic Blood Pressure Readings and What to Do About Them

You're probably familiar with the blood pressure cuff your doctor uses but do you really understand what he is measuring?

Blood pressure is the force that the heart places on the walls of your blood vessels when it pumps and relaxes. When the heart pumps the pressure goes up and this is called systolic pressure. When the heart relaxes, the pressure goes down and this is know as diastolic pressure.

A normal blood pressure reading is considered to be 120 systolic and 80 diastolic. If your diastolic is between 80 and 100 you are pre-hypertensive. If it is over 100 you have hypertension and will probably be placed on medications.

If you have a high diastolic reading you probably have a high systolic reading as well. It really doesn't matter though because high diastolic pressure carries the same life threatening conditions as systolic.

High diastolic blood pressure is more common in young adults, teens and even children. It's been suggested that birth weight may have a direct connection to this condition.

The three principal cause of high diastolic pressure are:



Obesity: Overweight people with a high Body Mass Index place additional pressure on the arterial system creating higher blood pressure. Excess fat and cholesterol can stiffen the arteries and clog them with plaque causing the heart to have to pump harder.

Diet: One of the biggest contributors to HBP and the principal cause of obesity, is an unhealthy diet rich in salt, saturated fats and trans-fatty acids. A diet with these characteristics can damage blood vessels at the cell level. This diet encourages oxidation which weakens cells, hardens arteries, and creates plaque.

Smoking: Putting nicotine in your body is not a good idea. Nicotine constricts the blood vessels increasing blood pressure and also causes the heart to beat faster.

So if you are an overweight smoker who thinks Burger King is gourmet food, you are probably in real danger.

The obvious solution is to set aside time to exercise. Thirty minutes of walking or cycling will have an immediate impact on your blood pressure ading as well as exercise the heart muscle and arteries.

You probably don't want to yell "I have blood in my poop!" when you're standing in a crowd. However, if you turn around one day and see bloody stools in the toilet, it's certainly something you might be screaming inside your head. Seeing blood is bad enough, seeing your own blood is worse. When you start seeing blood in your poop, that's gone too far. If you've just exclaimed to yourself, "I have blood in my poop!", what could be the problem? What should you do?

There are actually quite a few reasons why you might have blood in your poop. Some of them could be serious. Most of them are not. The first thing you need to ask yourself is, "How do I know it's blood?" You may not have realized this before, but there are actually several foods that can make your poop look bloody. Tomatoes and beets can turn your stools red. That makes sense now that you think about it, doesn't it? Other possibilities include blueberries, licorice, iron supplements, Pepto-Bismol--all of these can turn your stools black, which looks a lot like blood in your poop. First of all, when you received your type 2 diabetes diagnosis your blood sugar levels were too high, and then for reasons you are not too sure about yet, they seem to be all over the place. You don't understand why because you eat the same foods!

It is only when your levels rise too high, or sink too low, that blood sugar has serious consequences for your:


mood
weight
energy level
and maybe even your life

We are all different ... there is not one eating plan to suit all people with type 2 diabetes. How your body and your blood sugar respond to certain types of food, activity, stress and illness will also be different. This is one of the reasons you might like to work closely with a dietitian so your eating plan can be individualized.

An effective method to keep track of your eating habits and all that can affect your blood sugar level, is to create a food diary where you would include your:


blood sugar reading records
menstrual cycle dates
illness
stress, or
shift changes at work

Show detailed information, for example if you nibbled on meal ingredients as you were putting your meal together, if you had a snack while preparing dinner. A week of gathering this information will reveal a little more than you think!

Some days you think you eat the same as on other occasions, but here are some factors you may not always take into account:

food combinations ... did you eat two different types of fruit, for example one small apple as opposed to a large banana, or did you eat a smaller amount of protein this time with your carbs

health ... changes such as illness, menstrual cycle, pain and other factors can affect your blood sugars

stress ... your blood sugars respond to physical and mental stress. Work or relationship problems can cause stress which will cause an elevation in your blood sugars

alcohol ... alcohol interferes with your metabolism for several hours giving you lower blood sugar levels

blood sugar level checks ... are you doing these checks at the same time? Timing is everything ... if you are checking 90 minutes after eating one meal, and then 3 hours after eating the next ... well that would not be a true comparison

expired strips ... your readings will also be affected if you are testing with expired strips. Not getting enough blood will also affect your reading.
